Staff were very helpful, kind and friendly.

I would give zero stars but here we are. My mom was referred here by the hospital for rehab to build her strength up. The pics on the website are by far totally opposite of what is in person. Before she even arrived at the facility I was told the hospital called 3 to 5xs to give report but phone kept ringing. Upon arrival my moms transport ride got there before myself..I went to door of facility where understandably it was locked but when I pressed or rang the nurses button repeatedly no one answered. I called the facility also with no answer. I’ve been in Healthcare for 20 years this is unacceptable. The ambulance people came to door also trying to leave facility but couldn’t get out..a worker noticed and punched in the code to let them out but didn’t even ask who I was..I just walked in. My mom was in a two person room with pool noodles wrapped around and taped together around the railings which I guess was for previous patient. The blinds were all broken literal pieces broken off. I couldn’t distinguish from a tech or nurse. They did a skin assessment, asked some questions, took some vitals and was gone. Another tech brought water. No wheelchair, walker or any type of chairs/recliners was in the room..for it to be rehab you would think they’d have those things. I went to nurses station to ask for code and they said they don’t give it out which is weird considering you don’t answer the call from the door to get in. I pushed the light to have some one bring oxygen for my mom bc they had not yet. They brought a portable tank that didn’t work. To my knowledge when my mom said she was leaving not staying is when someone else came into to talk to her. The pics are very very misleading. This was not a rehab room. Very dirty and dated. I called to try to speak with the administrator or DON but got no phone call back. Terrible place. DON’T TRUST THE PICS!! THEY ARE 1 STAR RATED ON INSURANCE REFERRALS.

My mother-in-law has been here for two weeks now And so far, we have been very pleased with quality of care she’s received, the activities available, and the quality of the food. I will say, that we live nearby, so we are able to visit her once or twice a day, which has allowed us to stay on top of her care as well. The nurses, text and almost everyone we have encountered that works at this facility, have been incredibly helpful, kind, and responsive.

I wish I could give zero stars but I want to use this review as a warning to anyone considering this facility. My father was discharged post hip replacement to this facility and from the first few minutes I could tell this was going to be bad. The rooms are falling apart so much so, I had to clean the tall cabinet due to urine sitting in the bottom of it. The nightstands are missing parts of the drawers, the blinds are broken and bathroom was not cleaned. When the day shift leaves this place turns into one of the most dangerous places a loved one can be. We were told to label everything because things go missing easily (comforting right). There are drugs sold inside and outside of the facility, smoking inside, inappropriate movies being watched and residents roaming the halls. They are so short staffed that he was left soiled for multiple hours at a time on a regular basis. We discharged him to home healthcare after 4 days because I could tell he was frightened and his mental health was decomposing rapidly. He had also only had PT for 1.5 hours total in those 4 days. Upon arrival home today I discovered that he was being overmedicated and taking medication that was not his. Example – 2 different diabetes medications at the same time (his glucose was checked the days of admission and never after), high blood pressure medication (he actually has low blood pressure) and had been given 30 muscle relaxers in that 4 day time frame when it was supposed to be 1 PRN (or where they given to other residents?) and a medication that was in his records from 3 years ago that had not been prescribed or taken for over 2 years. Oddly enough when we made them aware that his was discharging he got a visit from admissions for the first time on the day he was leaving. While I understand some of this is just the nature of the business, this is not okay by any standard of care that one would expect from a place meant to help someone get better.

The nurses and CNAs were kind and caring to my loved one. They helped us reach out to Hospice and guided us through the process. I called at 3am because I was worried and couldn’t sleep. The team answered and gave me an update. ❤️

Great place very secure so patients can’t this walk out and get lost everyone was nice and took care of everyone’s health and needs

I am very Pleased with tha Facility and the Wonderful People that work there. Everybody, incl kitchen help, cleaning Crew are wonderful! Continue getting her out of bed and making her eat.

Place was very clean and lively. Seemed to be very well staffed. Staff was also very friendly and helpful.

I think this facility has a staff that works well together. If there are concerns raised. Those concerns are addressed in person by staff that is trained in that area. I is done in a timely manner. There are plenty of activities to keep you busy and productive. If I had to be in a long term care facility it would most definitely this one. The therapy department is top notch. This is a great place.

My John boy was discharged from the hospital 6/16/26 and admitted in Cedar Creek and 7/13/26 I called 911 myself and had him sent back to the hospital worse then when he was discharged. The nursing department failed John boy horrifically. Due to their lack of medical attention John had diarrhea from the second day being there and left untreated I asked for bloodwork to be done and it never happened. If I wasn’t there every day he would just be left sitting in his own mess. This is what his insurance paid for here. Thank God i listened to my gut and came in early because I found John lethargic and cold to the touch. When I asked the Nurse practitioner to please check his vitals she said it wasn’t her pay grade, I wasn’t even shocked at this point. When another nurse came in and checked him his B/P was 86/ 48 I walked out and said I wanted him sent out immediately because they were going to send him out non-emergency. I ended up calling 911. John is severely dehydrated due to the diarrhea they neglected to treat and his kidneys are effected and he’s being treated for a bacterial infection that I also asked them to check on that they also neglected. LOOK ELSEWHERE DON’T PUT YOUR LOVED ON IN HERE.

At this point my head hurts and I’m exhausted trying to stand up for my loved ones rights, and dignity. He was so mad today because he was hurt while being taken to the shower. He has only been in here less than a month and so much has happened its been extremely difficult and very hard to deal with. I’m doing my absolute best trying to maintain my professional composure. Pt has gotten better and started listening, thats good. But the nursing staff is very challenging I thought I was being heard and getting somewhere but I have my reservations. Just because I don’t know what else to do he remains here, I’m sincerely trying to remain hopeful. It’s only been a month I tell myself. Whats making this hard is the CNAs attitudes maybe they have too much on them? I don’t know, I’m doing my best to keep calm. I’m not difficult I just know the law and his rights and I am his voice. Simple things, the basics aren’t privileges they are his human rights and dignity. I can go on and on. I’ll be watching over his care every day! Facility life is rough! I pray 🙏🏽

Staff are really nice and friendly. They are happy and helpful. The staff at the front desk is really sweet. She always open the front door for me when I take my brother in law outside and back inside. The residents who stay there are friendly and sweet too. I like to visit my brother in law Jerry Stinson there

Thank you for all the support, my mom recovered well and now she is at home doing well. Thanks to all the tech that helped her while she was in there.

My mom was there 3/17/2026 for 1 week during that week never used her personal CPAP for COPD which was on her nightstand as “they didn’t have an order”. She was seen by wound care nurse on 2nd night because she had severe rash on bottom. Was told her treatment and medication to use that was in her wardrobe. The next night the tech came in to change her for bed and had no clue what wound care Mom needed. I had informed them she had forgotten how to use silverware but could eat finger foods. She lost over 10 pounds. On 3/20 at noon I went in and informed them I wanted to transfer her and needed referral with her information sent ASAP because I was trying to get her moved that day. Info was not faxed until 6pm and all the info needed wasn’t sent. 0n 3/24 still didn’t have all info including CPAP order. They did not get that sent until 3/25 and I transferred her as their transport can’t take someone with dementia without someone with her. The only good thing i can say is the lady in PT was awesome with her and the tech that came in her room to change her for bed on 3/19 may not have known the orders but she listened to me and was very gentle with my mom. You could tell she cared.

Very un-oriented facility a lot of nurses and care givers have expired certifications out of date on a lot of their paperwork if the state were to come in the facility would be shut down a lot of the night shift workers are not working and nurses do not do their job to the fullest. Not satisfied

Definitely recommend coming here especially since they got new management. Cameron has been especially great to work with.
